Parallel systems do not scale
A non-profit centre can run an excellent programme in a district. It cannot run one in every district, and a programme that exists only while external funding lasts leaves a gap behind it when that funding ends.
Public health systems already have the reach. What they often lack is specific laboratory capacity, particular technical expertise, or the evidence base for a decision. Supporting existing public provision is more durable than building alongside it, so we design our work to strengthen public services rather than substitute for them.
